They contain your personal information and identity descriptors (such as Social Security numbers) that if stolen or misused, leaves you at serious risk of medical or other identity fraud. When you receive medical care at a clinic or hospital, you lend information about your identity to the organization and it is used to either enroll you as a patient or to access your existing medical records.

If the facility participates in the Identity Network, a small amount of the identity information you provided (your name, gender, date of birth, address, country of birth) and a biometric (fingerprint, iris, facial image, signature or voice) is registered in the Identity Network and two things happen:
